Eligibility
In-home vaccinations are available for anyone who has difficulty getting to or using a community vaccination location, including children 6 months to 17 years old.
How to schedule an in-home vaccination
By phone: Please call the In-Home Vaccination Central Intake Line at (833) 983-0485.
Representatives from CDR Health, which is operating the call center, scheduling appointments, and delivering in-home vaccinations are available at this number Monday through Friday from 9 a.m. to 5 p.m.
Representatives are available in both English and Spanish, and also have access to translators for over 100 other languages
